Patients
We ask that all patients complete One Medical Passport located under the Pre-Registration tab, prior to your procedure date. You will receive a call from our Pre Screening Nurse to review your medication list as well as your history and physical. Patients should not eat or drink anything the morning of your scheduled procedure. On your procedure date, we ask that you arrive at the time your physician office assigns you. Please do not bring any valuable items; the facility will not be responsible for any lost or stolen items. You are required to bring your photo identification, insurance card, payment and ride contact information.
Billing and Payments
We require payment of any copays, deductibles and coins at the time of admission or prior to. We will verify your benefits as a courtesy to you. If your insurance carrier require authorization and /or referral these must be obtained prior to your procedure date. We recommend that you call your insurance carrier to verify that we are a participating provider in your plan. Patients without medical insurance or procedures not covered by your insurance require full payment at the time of admissions. Pathology, labs and anesthesia are billed separately. Your co-payment and past-due account balance is due at the time of service. We accept checks, debit, all major credit cards and CareCredit. Cancellations or Rescheduling
We ask that you contact our office at least 24 hours in advance, if you need to reschedule or cancel your procedure.
After-hours & emergencies
If you are experiencing a medical emergency and require immediate care, please dial 911. If you are not experiencing a medical emergency and need medical advice, contact your physicians office.
