Please note that your benefit level is an agreement between you and your insurance company. If any additional payment is necessary, this is your responsibility. Likewise, if the insurance company does not pay, the patient is the guarantor of payment.
If you are paying with cash or credit card, please make arrangements in advance. Payment and/or deductibles are due at time of service.
